You may have been directed to this page to complete an online medical admission  prior to your visit.Your information will be securely held at our practice and is not stored on the internet.

We request that all patients complete their online admission prior to arrival to enable a smoother check-in and less paper work for you, our patients. New or existing patients might prefer to complete our patient registration form instead. Please let our reception staff know at the time of booking your appointment or upon arrival to our centre.

Existing patients of the Digestive Health Centre, are advised to update your medical conditions, medications and admission details such as medicare card etc.. prior to your visit.

The online admission may take up to 15 minutes to complete depending on your medical conditions and medications.
